Cancer care involves more than medical treatment alone. Our care coordination team is here to guide and support you throughout your entire care journey—helping ensure you receive the right care at the right time.
Ongoing care coordination is provided by experienced oncology clinicians and dedicated support staff who work closely with you during treatment and beyond. Your care coordinator serves as your primary point of contact, helping you manage not only your medical needs, but also the physical, emotional, and practical challenges that may arise along the way.
Our care coordination services include:

Survivorship Care
Your care doesn’t end when treatment does. Our survivorship program supports your long-term health and quality of life after active cancer treatment.
We provide individualized survivorship care plans, ongoing monitoring, and guidance to help you transition with confidence. Our team also connects you with community-based resources that support physical wellness, emotional health, and survivorship-related needs—so you feel supported every step of the way.

Meet Your Oncology Dietitian

Hillary Buch, MS, RD, CSO, LDN
Hillary is a registered dietitian experienced in oncology nutrition. After receiving a bachelor’s degree in clinical nutrition from the University of California Davis, Hillary earned a master’s degree in nutrition and dietetics from Saint Louis University. She worked closely with lung and esophageal cancer patients at Barnes-Jewish Hospital in St. Louis, Mo. and worked with a variety of cancer types at Moffitt Cancer Center in Tampa, Fla.
Hillary provides personalized nutrition plans for each patient, helping to find the right foods during treatment to lessen the side effects and improve recovery. She also specializes in treating and preventing malnutrition. Hillary enjoys helping each patient to feel their best and meet their nutrition-related goals.
